We are looking for an enthusiastic, compassionate and motivated Band 6 Community Mental Health Worker to join our Adult Community Mental Health Treatment Team. This is an exciting opportunity to become part of a multidisciplinary team that puts people at the heart of everything we do. You will work alongside service users, carers and partner neighbourhood agencies to provide high-quality, recovery-focused care to people experiencingserious and enduring mental health difficulties. Main duties of the job As a Key Worker, you will have a central role in supporting people on their recovery journey. You will: Build meaningful therapeutic relationships with service users and their carers. Complete comprehensive assessment, care planning and risk management. Coordinate and deliver evidence-based, person-centred interventions. Work creatively with individuals to identify their goals, strengths and aspirations. Work collaboratively across the multidisciplinary team and wider partner agencies to ensure care is coordinated and effective. Provide professional advice, guidance and support to colleagues and external agencies. Use your experience and clinical expertise to support, coach and develop junior and less experienced staff. Act as a positive clinical role model, contributing to a culture of learning, innovation and high-quality care. Why join us?
