Job summary HASP PCN is a network of five GP practices across Hull. The PCN Pharmacist will work as part of the PCN overlay team providing services to all member practices. The Primary Care Pharmacist (Independent Prescriber) will work as part of the multidisciplinary team (MDT) within the Primary Care Network to deliver high-quality, patient-centred clinical pharmacy services. The post holder will undertake structured medication reviews, support the management of long-term conditions, provide prescribing support, contribute to Quality and Outcomes Framework (QOF) achievement and assist in the delivery of PCN Directed Enhanced Service (DES) requirements. The post holder will support medicines optimisation, improve patient outcomes, reduce health inequalities, and contribute to the delivery of national and local healthcare priorities.
