Job summary As a Clinical Pharmacist, you will play a key role in delivering high-quality patient care and medicines optimisation services. You will work closely with our GPs and wider clinical team to support patients with long-term conditions, improve prescribing safety, and contribute to the ongoing development of our services. Main duties of the job Key responsibilities will include: Conducting structured medication reviews. Supporting patients with long-term management. Managing medication-related queries and prescribing requests. Undertaking medicines reconciliation following hospital discharge. Supporting safe and cost-effective prescribing. Working collaboratively with GPs, nurses, pharmacy technicians, and community pharmacy colleagues. Contributing to quality improvement, clinical audits, and medicines optimisation initiatives. Good understanding of patients role in decisions about prescribed medicines and supporting adherence as per NICE guidance Excellent interpersonal, influencing and negotiating skills Excellent written and verbal communication skills Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g., patients) Ability to plan, prioritise, manage, monitor, advise and review clinical and general medicine optimisation issues in core areas for long term conditions.
