Job summary The Uplands Medical Practice is seeking a motivated and experienced Advanced Pharmacist Practitioner to join our multidisciplinary primary care team. This is an exciting opportunity for a clinically focused pharmacist to play a key role in improving patient outcomes, enhancing medication safety, and supporting the delivery of high-quality care across the practice. The successful candidate will work closely with GPs, nurses, and other healthcare professionals to provide expert pharmaceutical care, support chronic disease management, and contribute to regulatory compliance and quality improvement initiatives. Main duties of the job The Advanced Pharmacist Practitioner will provide comprehensive clinical pharmacy services within the practice, including conducting structured medication reviews, managing prescriptions and repeat prescribing processes, implementing medication safety alerts and MHRA guidance, and supporting safe, effective, and cost-efficient prescribing. The role will also involve supporting patients with long-term conditions such as diabetes, hypertension, asthma, and COPD through medication optimisation, monitoring, treatment reviews, and patient education. In addition, the post holder will review and manage blood test results related to medication monitoring, contact patients regarding follow-up actions where required, and ensure compliance with clinical safety standards. The successful candidate will assess and manage minor ailments within their scope of practice, providing treatment advice, prescribing where appropriate, and making referrals when necessary. They will also play an important role in supporting the practice's quality, safety, and governance agenda by contributing to CQC compliance, clinical audits, quality improvement projects, medicines management initiatives, and the development of policies and protocols to ensure the delivery of high-quality patient care.
