Job summary This exciting role is based within the Urgent Community Response Team. The service provides urgent care to people in their homes which helps to avoid hospital admissions and enables people to live independently for longer. By referring to the Urgent Community Response team, older people and adults with complex health and social care needs who urgently need support, can get fast access to a range of health and social care professionals within two hours. Working as part of the integrated multi-disciplinary team. You will provide therapy assessments and urgent intervention, advice and support in the management of patients in the Warrington area. Urgent Community Response is a service delivered in collaboration with our local authority partner. The post holder will undertake comprehensive assessments of patients within their own homes with acute/unstable conditions who are referred to the Urgent Community Response Service as an alternative to hospital admission. Actively participate and contribute to multidisciplinary team working also liaising with other professional agencies for on-going support and long-term review. Main duties of the job To undertake comprehensive and holistic therapy assessments of patients including those with diverse presentations, multiple pathologies and complex needs, in a variety of community settings. To utilise advanced analytical skills and clinical reasoning for people in a health or social crisis. Utilise assessment findings and treatment outcomes to develop and coordinate appropriate, timely discharge plans, ensuring safe transitions of care, admission avoidance and referral to relevant services to support on-going needs. To assess and treat patients, in their own environment, without prejudice and in conditions hazardous to health with the potential to cause harm. Be able to work flexibly in accordance with the needs of the service. Work over a 7 day period. About us Flexible working will be considered for all roles.
