Job summary Due to retirement, we are seeking an exceptional clinical pharmacist to support the practice team and patients. What we are offering: A competitive package. Main duties of the job Conducting comprehensive medication reviews and optimising medication regimens Provide advice on medication management to patients, healthcare professionals, and the multidisciplinary team. Support the management of chronic diseases through medication adjustments and monitoring after discussion with medical team. Corresponding with secondary teams if clarification needed of prescribing plans. Discharge medication reconciliation. Support health board prescribing incentive schemes, and clinical audits.
