You will assess patients’ nutritional needs; develop, implement, and monitor care plans; and ensure compliance with federal, state, and center guidelines. Success in this role requires strong communication, food systems management knowledge, and interdisciplinary collaboration. Responsibilities Complete and document admission, quarterly, annual, significant-change, and high-risk patient nutrition assessments Develop and monitor individualized nutrition care plans Provide nutrition education and training to food service and center staff Collaborate with nursing, therapy, administration, and food and nutrition services to promote optimal patient outcomes Participate in care plan meetings and interdisciplinary committees Lead Quality Assurance and Performance Improvement (QAPI) initiatives, conduct audits, and implement action plans for continuous improvement Complete MDS assessments and documentation Qualifications Bachelor's or Master's degree in Nutrition or Dietetics from an accredited college or university Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR) Current RD licensure Knowledge of long-term care regulations and nutritional standards Strong organizational, communication, clinical, and interpersonal skills Basic computer and documentation skills Schedule In-person, Monday–Friday (80 hours bi-weekly), with flexibility within the week.
