Job summary Under the direction of the Head of Service, Healthcare Support Workers will assist in delivering high-quality, person-centred care and support to patients with life-limiting illnesses, ensuring their comfort, dignity, and well-being. Shortlisted applicants will be invited to an open day being held on 20th February 2026. Main duties of the job Provide compassionate and empathetic care and support to patients with life-limiting illnesses and their families, respecting their dignity, choices, and cultural preferences. Assist patients with personal care tasks, including bathing, toileting, dressing, and feeding, ensuring their physical comfort and well-being. Support patients with mobility, transfers, and positioning, using safe manual handling techniques and equipment as required. Facilitate late discharge and support overnight admissions into hospice care. Monitor patients' vital signs, symptoms, and general condition, reporting any changes or concerns to the Head of Hospice at Home. Document patient observations, care interventions, and other relevant information accurately and timely in accordance with hospice policies and procedures. Assist patients with activities of daily living, including meal preparation, serving, and feeding, ensuring their nutritional needs are met and dietary preferences are respected. Support patients in engaging in recreational activities, hobbies, and social interactions to promote their well-being and quality of life. Maintain a clean, safe, and comfortable environment for patients, ensuring their rooms and communal areas are tidy, well-stocked, and free from hazards.
