Clinical Pharmacist

Job summary As a Clinical Pharmacist at Marlow Medical Group, you will provide clinical pharmacy expertise across the practice, supporting safe, effective and sustainable primary care delivery. You will work closely with GPs, nurses, advanced practitioners, pharmacy colleagues, administrative teams and the wider multidisciplinary team to improve patient outcomes, support medicines safety, reduce avoidable hospital admissions, improve access to care and strengthen medicines optimisation across the organisation. You will use your clinical expertise to support patients with long-term conditions, minor ailments, medication queries, structured medication reviews and post-discharge medicines reconciliation. You will also contribute to prescribing safety, repeat prescribing processes, quality improvement, clinical governance and the delivery of relevant primary care and PCN priorities. Main duties of the job The Clinical Pharmacist will provide direct care to patients with long-term conditions, minor ailments and other clinical needs within their professional scope of practice. You will undertake medication reviews and structured medication reviews, either face to face, remotely or, where appropriate, in a patients place of residence. These reviews will be centred on each patients individual needs and will support safe, effective, evidence-based and person-centred prescribing.
