Advanced Paramedic Practitioner

Job summary We are seeking an experienced and highly motivated Advanced Paramedic Practitioner (APP) to join our multidisciplinary team at Farnham Park Health Group. The role will focus on providing high-quality patient care by supporting clinical staff in managing acute and chronic conditions, undertaking home visits, and contributing to Quality and Outcomes Framework (QOF) achievement work. The successful candidate will play a key role in reviewing blood test results, prescribing medications within their competency, and supporting administrative functions to ensure effective patient management. We have a weekly clinical meeting with representation of all members of the practice. You will have an interest in primary care, triage, minor and chronic illness practice work. This role is pivotal to improving the quality of care and operational efficiencies so self-motivation and passion to deliver excellent service within general practice is essential. Main duties of the job Main Duties: - Be the first point of contact for patients presenting with undifferentiated, undiagnosed problems. Making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan. -Refer patients to alternative care setting where appropriate. -Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports. -Provide patient education on managing health conditions and preventative care. -Ensure clinical correspondence is appropriately managed. We are part of Farnham Integrated Care Services working in collaboration with local practices offering same day access for urgent care along with MSK Practitioners and Home Visiting Services. X-ray, ultrasound and phlebotomy services available within Farnham Centre for Health. We are also a Training Practice. CQC rated 'good'. 6 weeks annual leave pro-rata and competitive remuneration package, dependent on experience.
