Job summary An exciting opportunity has arisen for a highly motivated and enthusiastic Mental Health Practitioner to join the Community Mental Health and Memory Service. Main duties of the job Our ideal candidate would be a committed team player as well as being able to work independently, demonstrating initiative and be passionate in promoting good quality care to maximise independence. Within this role you will be responsible for assessing, developing, implementing and evaluating care plans using a person-centred approach. Within this role you will gain training, support, and guidance to ensure you are working collaboratively with external agencies to provide the best possible care for our service users. This role requires someone who can manage in a fast-paced environment completing assessments to support diagnosis. The day-to-day work - for this role will be led by the needs of the service users and the service which will require effective time management and prioritisation skills as well as effective communication and autonomous working. This involves being responsible for assessment, developing, implementing and evaluating care plans using a person-centred approach. As a member of a community mental health and memory service multi-disciplinary team, be involved in liaising and effectively communicating with all relevant parties including GPs and carers. To promote, innovate and create ways of working with service-users who have acute, complex and longer term mental health problems. 1. Communication & relationship skills Use communication skills to conduct assessments of service-users and communicate complex information related to their mental health needs, to service-users, carers, relatives and other professionals. Effectively communicate complex and/or sensitive information where there are barriers to understanding. This includes communication concerning adult protection, abuse, cognitive impairment and other risks surrounding a service-users mental health. Barriers to understanding include: working with service-users who could be hostile or highly emotional, who have complex needs and may be vulnerable. Maintain and develop effective communication and working relationships within the team and with external agencies. Promote and implement creative ways of working with service-users. Instigate, develop and maintain a relationship with the client and / or carer to facilitate the accurate assessment and delivery of care. Manage highly confidential information in respect of members of the public in accordance with local and national policy. Act as care co-ordinator and named worker responsible for specialist assessment, reviewing and on-going care. 2. Knowledge, training and experience The post-holder will hold current registration on part 3 or 13 of the NMC register or hold a Registered Occupational Therapist, Social Worker, Mental Nursing (RMN) certificate.
