Job summary Band 5 RMN / OT / Social Work Professional Exeter City Core Community Mental Health Team (CMHT) Full-Time Substantive Post (37.5 hours per week) Flexible / Hybrid Working Available We are seeking enthusiastic and compassionate Band 5 Registered Mental Health Nurses (RMNs), Occupational Therapists (OTs), or Social Work professionals to join our Exeter City Core CMHT a team known for its dynamic, innovative, and supportive culture . Main duties of the job Main Duties & Responsibilities Deliver high-quality, person-centred care to adults with complex mental health needs in the community. Work collaboratively within a multidisciplinary team to develop and implement care plans. Conduct assessments, risk evaluations, and reviews in line with clinical governance and safeguarding protocols. Provide therapeutic interventions and recovery-focused support. Liaise with service users, carers, and external agencies to ensure continuity of care. Maintain accurate and timely clinical documentation. Participate in team meetings, supervision, and ongoing professional development.
