If you meet these qualifications, we would love to meet you: Must be a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in the state(s) of practice, with training or experience in caring for chronically or terminally ill patients Maintains active DEA license (Required) Experience in palliative care Working knowledge of hospice and palliative care, including pain and symptom management. Familiarity with hospice regulations and Medicare requirements. Strong clinical judgment and decision-making skills. Excellent communication skills for interaction with patients, families, and providers. Ability to manage complex clinical and ethical situations in end-of-life care. (Preferred) Fellowship, Certification, etc. in Hospice and/or Palliative Care Job Responsibilities: Develop and maintain Plan of Care for each patient, including discharge planning discussions with care team, patients, and families Participate in prognosis evaluation and certification/recertification Sign death certificates as needed Conduct daily patient rounds and set consistent rounding schedules with interdisciplinary team Ensure proper documentation of eligibility for inpatient care Provide education and training to interdisciplinary team and new staff Why AccentCare?
