Job summary Are you a compassionate Healthcare Assistant who enjoys making a real difference to people in their own homes? An exciting opportunity has arisen to join our Urgent Care Response and Intermediate Care Team, supporting patients across the community in North Kent. Our service plays an important role in helping to prevent avoidable hospital admissions, support timely hospital discharge and enable people to recover and maintain their independence at home. Working alongside Nurses, Therapists and other members of our multidisciplinary team, youll provide hands-on care and rehabilitation support to patients with a range of needs. This could include supporting someone with their rehabilitation programme, helping them regain confidence with everyday activities or providing short-term personal care while longer-term support arrangements are put in place. Youll work independently on delegated activities while remaining fully supported by experienced clinical colleagues. With access to mobile technology such as tablets, youll be able to work effectively while out in the community and maintain accurate and timely patient records. This is Main duties of the job As a Healthcare Assistant in our Urgent Care Response and Intermediate Care Team, you will support the delivery of individual care and rehabilitation programs to patients in their homes. Work with the team to help patients regain independence and remain safely in the community. Youll perform delegated nursing and therapy activities as directed. Youll support patients with mobility, independence and rehabilitation activities, provide short-term personal care, and aid in the timely discharge of patients from hospitals. Observing patients closely and maintaining timely patient records is crucial. Building positive relationships with a diverse range of patients and families is essential, along with enhancing your skills through learning and development opportunities.
