Job summary ASpecialist Community Public Health Nurse in HealthVisitingtoprovidestrong clinicalcare andleadershipwithinan integrated skill mixed teamas part of the0-19 Universal Health Service in Blackburn with Darwen. To lead on the delivery of the Health Child Programme 0-5yearsand defined local requirements based on the provider contract. To provide family centred, evidence based public health service thataims to proactivelyimprove healthoutcomes for a defined caseload,populationorcommunity, reducingthe risksand impactof health inequalitiesby working in partnership with families,communitiesandother professionals.This will be achieved through delivery of the model of health visiting at acommunity,universal,targeted andspecialistlevel.Safeguarding is embedded within themodelconsideringsafeand effective managementofriskforchildren, youngpeopleand families. More than 1 position is available. Full time and part time.
