Community Mental Health Practitioner

Job summary We are looking for (0.8 WTE) experienced Mental Health Practitioners, from a professional background of Nursing, OT or Social Work who can demonstrate excellent assessment and communication skills, in working with people in the community experiencing a mental health crisis and the ability to deliver a compassionate, person-centered and high quality service to patients, carers and others who are referred to the Neighbourhood Mental Health Team. You will need to have experience of assessing and managing risk in mental health. Main duties of the job The service is looking to appoint candidates who are highly motivated, enthusiastic, creative and resourceful, and who have a passion for working with people and for developing good working relationships with carers. You will have the opportunity to forge close links with the third (voluntary) sector, as well as with other specialist teams within the Trust and within Primary Care and Partnerships.
