Main duties of the job The successful candidate will provide expert pharmaceutical care, supporting patients across a range of clinical settings. Main duties of the role will include: Conducting patient-centred structured medication reviews to optimise medicines use and improve health outcomes. Providing higher-level clinical administration and medicines management support to ensure safe and effective prescribing practices. Managing and supporting the ongoing care of patients with long-term conditions, using your independent prescribing qualification where appropriate. Identifying and addressing medicines-related issues, improving patient safety and reducing unnecessary hospital admissions. Working collaboratively as part of a multidisciplinary team (MDT), contributing your specialist knowledge to support holistic patient care. Supporting the delivery of national and local clinical priorities, including prescribing quality initiatives and service improvement projects. Building strong relationships with patients, carers, and healthcare professionals to promote shared decision-making and person-centred care.
