Job summary Package Description: As a Healthcare Assistant, you will join our valued Gravesham Central Community Team based at Gravesham Community Hospital. This role is ideal for someone who enjoys working out in the community rather than being based in one location all day. You will build meaningful relationships with patients and play a key role in supporting their health and wellbeing in a more personal, one to one setting. You will be part of a supportive, close knit community nursing team that works together to deliver high quality, compassionate care across the Gravesend area. You will be joining a welcoming and experienced community nursing team who are passionate about delivering personalized care closer to home. The role is community-based, ideal for those who enjoy working beyond a single location each day.
