Job summary <p>Are you a compassionate Healthcare Assistant who enjoys making a real difference to people in their own homes?</p><p>An exciting opportunity has arisen to join our Urgent Care Response and Intermediate Care Team, supporting patients across the community in North Kent.</p><p>Our service plays an important role in helping to prevent avoidable hospital admissions, support timely hospital discharge and enable people to recover and maintain their independence at home.</p><p>Working alongside Nurses, Therapists and other members of our multidisciplinary team, youll provide hands-on care and rehabilitation support to patients with a range of needs. This could include supporting someone with their rehabilitation programme, helping them regain confidence with everyday activities or providing short-term personal care while longer-term support arrangements are put in place.</p><p>Youll work independently on delegated activities while remaining fully supported by experienced clinical colleagues.
