Job summary A fantastic opportunity has arisen for a fully qualified and experienced ANP or Paramedic Practitioner to join our friendly and supportive team at Stamford Hill Group Practice. Main duties of the job You will assess, diagnose and treat patients presenting with minor illnesses, providing advice and ongoing support for their continuing healthcare needs through both face-to-face and telephone consultations. Home visits are an essential part of this role. You will deliver a high standard of patient care as part of a multidisciplinary team of clinicians. This role requires a comprehensive clinical knowledge base, with the ability to demonstrate safe clinical decision-making and provide expert, patient-centred care. You must be able to work both autonomously and collaboratively as part of the wider practice team. Undertake comprehensive clinical assessment, diagnosis, treatment, management and evaluation of patients presenting with undifferentiated and undiagnosed conditions. Independently manage a wide range of acute, chronic and urgent health conditions within scope of practice. Conduct face-to-face, telephone and home visit consultations as required. Develop and implement appropriate management plans, including medication reviews and referrals to other services where necessary. Request, interpret and act upon diagnostic investigations, including blood tests, ECGs, imaging and other relevant investigations. Recognise serious illness and clinical deterioration, ensuring appropriate escalation and referral to secondary care or emergency services. Provide advice on health promotion, prevention, self-management and lifestyle modification. Support the proactive management of long-term conditions in accordance with local and national guidance. Contribute to reducing health inequalities and improving outcomes for vulnerable patient groups.
