Job summary Are you a compassionate Dietitian with experience of working with people with mental illness? Join Thornford Parkas a Dietitian. You will be responsible for supporting the delivery of the dietetic service across the hospital, supporting the ongoing implementation of the Hospitals Nutrition and Hydration Strategy; helping to enable people to make informed healthier choices more easily when it comes to eating and drinking. With experience in managing a caseload using theoretical and practical experience, you will be involved in completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans. The key responsibilities include supporting service users in a secure mental health setting and contributing to the hospitals Nutrition and Hydration Strategy. You will work both individually and as a team member to deliver dietetic services, involve in planning interventions, and evaluate outcomes. As a Dietitian, you will be: Being involved and active in the development of a need led service. Providing dietetic clinical services both on an individual and group basis. Completing assessments, plan, implement intervention and evaluate outcomes as per individual service users care-pathway, enabling the service users in areas of self-maintenance, productivity and leisure. Planning and implementing individual and group interventions collaboratively with patients. Evaluating outcomes aligned to care plans. Maintaining accurate patient records, including care notes, written reports (as appropriate) following hospital and professional standards. Establishing effective communication with patients and their carers, all staff and other agencies.
