Job summary This role sits within the SMI pathway of the Specialist Forensic and Community Wellbeing Team and helps bridge the gap between primary and secondary mental health care by providing accessible, primary care-level mental health interventions for people in North Lincolnshire. The service is committed to strengthening links with Primary Care Networks, with colleagues already embedded in GP surgeries, and further development of this work is a key priority. Main duties, This is an exciting opportunity for an experienced, enthusiastic and organised mental health professional to join the team as a Senior Support Worker. The post involves delivering high-quality healthcare support under the direction of the Nursing Team and Allied Health Professionals within a primary care setting. The role will focus mainly on the SMI pathway, completing physical observations and physical health checks, while also supporting adults experiencing mental health difficulties. The postholder will work across pathways as needed and contribute to integrated, person-centred interventions in line with the primary care model. Main duties of the job The post holder will support the Specialist Forensic and Community Wellbeing Team, primarily within the Severe Mental Illness pathway and, where required, the Medication Management pathway. Working under the supervision of the nursing team, they will assist with assessments, investigations, care planning, delivery and review of interventions in line with relevant policies and pathways. Key duties include supporting clinics across North Lincolnshire, completing annual physical health checks, promoting health and wellbeing, providing education and advice, and signposting patients and carers to appropriate services. The role involves supporting patients with health promotion activities, monitoring wellbeing and signs of relapse, contributing to MDT discussions, and helping patients manage issues such as complex medication and physical health needs. The post holder will maintain accurate paper and electronic records, including waiting lists and SMI registers, and report risks or changes in patient presentation promptly.
