Job summary An exciting opportunity has become available at Hedena Health to join our Integrated Neighborhood (Proactive Care) Team as a Paramedic or Advanced Practitioner. This team is known as the PACT (Proactive Care Team). We are seeking an enthusiastic and motivated Paramedic/ANP to join our friendly and supportive team establishing an innovative multi-disciplinary way of working that aims to improve the experience and outcomes of primary health care for the most complex patients at Hedena. The project aims to provide an internal multi-disciplinary team (MDT) that brings together GPs, GP trainees, Practice Nurses, Paramedics, Pharmacists, Social Prescribers, Community Health and Wellbeing Workers and a Mental Health Nurse as needed to integrate proactive care for the most vulnerable or at risk of unplanned hospital admission or rapid deterioration of their health. Main duties of the job The PACT team further integrates with wider external health and social care networks via weekly MDT meetings within our PCN (Manor surgery) which connect with Oxford City Council, social services, housing services, mental health services, district nursing, OUH care of the elderly and ED services as well as urgent hospital at home services. This new and innovative approach to patient care, and our ethos of team working and personal/career development, aims to improve health care for the most at need. The Paramedic/ANP role is one of direct patient care either through telephone assessments, face to face assessments, and will include a significant element of home visiting. This will not replace acute PCVS home visiting service or district nursing service but will complement this and work alongside these services. The Proactive Care Team work proactively to improve access to healthcare for the practices highest need patients and their families. The team aim to provide support for patients and their families in the community in their own home environment through both short-term health and social care crisis. This can be during acute illness, chronic condition treatment at home, longer-term frailty, end of life care and to help prevent hospital admission. The team provides high quality, person centered clinical care for those living with complex health and social needs, addressing individual needs within a patients own environment. This is achieved by collaborating with all health and social care agencies and integrating with other services across primary and secondary care.
