Location: Legacy Acres Homes - 3011 W Gulf Bank Rd, Houston, TX 77088 Hybrid Registered Dietitian - Job Overview Schedule: 8AM-5PM | Hybrid Join our Legacy Community Health family as a Registered Dietitian and use your passion for nutrition to make a meaningful difference in the lives of pediatric and adult patients. In this role, you will apply proven nutrition science to assess, counsel, and support patients in preventing and managing diet-related diseases and addressing other nutritional concerns across every stage of life. You will build trusting relationships with patients and families, create individualized diet plans, provide practical nutrition guidance, and monitor behavioral and biometric progress. Your care will account for complex medical conditions, age, growth, normal development, and the diverse cultures and backgrounds of the people we serve. Help patients and families turn nutrition guidance into achievable, meaningful health goals. Support health promotion, disease management, weight control, and care for complex and acute nutritional concerns. Collaborate with an interdisciplinary team and consult with providers regarding patient diagnoses and progress. Contribute to community healthcare by helping patients access culturally responsive, whole-person nutrition support. Participate in quality improvement, education, and initiatives focused on diet-related illness. Position may be fully or partially funded by grant(s). Key Responsibilities Evaluate diet history, 24-hour recall, and food frequency data of pediatric and adult patients. Prepare diet plans for health promotion, disease management, weight control, and other conditions including complex and acute concerns. Assist patients to set SMART goals. Assess patient progress through the interpretation of laboratory data, body composition data, patient interview, and shared decision-making. Serve as a member of an inter-disciplinary care team and consult with various providers on patient diagnoses and progress. Document all patient services in the electronic health record and other data systems as required. Manage patient schedules and desktop in the electronic health record system. Maintain any equipment or instructional materials needed to conduct counseling sessions. Assure agency and contract requirements are met, such as for appointment volume, type, and billing. Collect and report monthly statistics and other reports as required. Direct the design of new initiatives related to diet-related illness and conduct evaluations of current efforts. Participate in the Quality Improvement Program and other performance improvement efforts as directed. Be available as a consultant to clinic staff concerning nutritional information and referrals for patients. Participate in nutrition team, provider team, clinic team, and departmental and agency meetings as needed. Attend training workshops and meetings as required to meet licensure requirements. If applicable, monitor assigned budget(s) for Units of Services and Nutritional Supplement spending. If assigned, direct support staff. Interview and advise patients regarding health and illness prevention, including FQHC-required voluntary family planning services and related preventative health services in compliance with funding requirements and state and federal laws, rules and regulations. Perform other duties as assigned. Perform other functions as outlined in the Dietitian Delineation of Privileges. Minimum Qualifications Bachelor’s Degree from an accredited university in dietetics, nutrition, or a related field required Initial Credentialing Minimum Criteria for Core Privileges: Must be a Registered Dietitian (RD). Must hold an active license in the state of Texas. Evidence of current competence within the last 12 months to perform privileges requested. Re-Credentialing Minimum Criteria for Core Privileges: Evidence of current competence within the last 12 months to perform privileges requested. Completion of required continuing education to maintain Texas licensure. Provider shall obtain, maintain and participate in the Medicare and Medicaid Programs, workers’ compensation, other federal and state reimbursement programs, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, and other health benefit program requested by Legacy. Required Years of Experience Master’s Degree or 2 years of experience working as a dietitian and/or providing nutrition counseling and education to clients. Required Knowledge, Skills, and/or Abilities Experience working as a dietitian and/or providing nutrition counseling and education to clients. Experience with computers and computer programs. Experience working in the area of nutritional needs for persons with chronic conditions such as diabetes. Desirable Knowledge, Skills, and/or Abilities Bilingual English/Spanish. Experience working in a healthcare system. Experience working in an electronic health record system. Experience working in the area of nutritional needs for persons living with HIV.
