Job summary Primary Care Sheffield are looking for a Mental Health Practitioner. This role provides skilled interventions to people with mental health problems in the Primary Care Networks. You will be working within a multi-disciplinary team as a member of the PCN level primary care team. Main duties of the job Management of own patient group in line with General Practice procedures and risk management protocols Provision of leadership to the Team in respect of psycho-social interventions to people and carers. Youll work as part of a multi-disciplinary primary care team , managing your own patient group and working closely with people, carers and other professionals to provide high-quality, person-centred care. What you'll be doing: Managing your own patient group in line with General Practice procedures and risk management protocols. Assessing needs, identifying problems and devising plans of care in partnership with people and carers. Implementing and reviewing planned programmes of care, taking appropriate action where required. Advising and supporting people and carers to promote health and wellbeing and prevent illness. Establishing and maintaining communication with all agencies involved in a persons care in both hospital and community settings. Functioning as a member of a multi-disciplinary team and providing leadership in respect of psycho-social interventions to people and carers. Maintaining accurate patient care records and managing your caseload with due economy of cost and time. Producing audits and participating in service development, clinical governance, research and clinical audit as required. Keeping the Management Team and colleagues informed of developments relevant to your area of responsibility. Supervising students and trainees on placement and assisting in the teaching and in-service training of staff. Keeping up to date with current trends in clinical practice and attending relevant courses and training. Promoting an understanding of mental health and wellbeing and ensuring health and safety regulations are observed.
