Main duties of the job As a Clinical Pharmacist, you will play a key role within our multidisciplinary primary care team, providing expert clinical pharmacy services to improve patient outcomes and support safe, effective prescribing. You will undertake structured medication reviews, manage long-term conditions, optimise medicines, and provide specialist advice to patients and healthcare professionals. Working closely with GPs, nurses, community pharmacies and secondary care, you will help ensure seamless medicines management across care settings. The role also includes supporting quality improvement initiatives, promoting evidence-based practice, contributing to service development, and providing education and support to colleagues.
