Physician Associate - Home Visit Service

Job summary Derbyshire Dales Primary Care Network are seeking a motivated and compassionate Physician Associate (PA) to join our dedicated Home Visiting Service . Working as part of a multidisciplinary primary care team, you will provide high-quality clinical assessment, diagnosis, management, and follow-up care to patients within their own homes. This is an exciting opportunity for a skilled clinician who enjoys autonomous working while remaining supported by experienced GPs and other healthcare professionals. You will play a pivotal role in improving access to care, reducing unnecessary hospital admissions, and supporting some of the most vulnerable members of our community. Main duties of the job Key Responsibilities Conduct comprehensive clinical assessments in patients' homes. Obtain and document detailed medical histories. Develop and implement management plans within your scope of practice. Request and interpret appropriate investigations. Identify deteriorating patients and escalate concerns appropriately. Prescribe through delegated pathways where locally agreed and in accordance with current regulations. Work collaboratively with GPs, nurses, paramedics, pharmacists, and community teams. Maintain accurate and timely clinical records. Participate in clinical governance, audits, and quality improvement initiatives.
