Clinical Pharmacist

Job summary We are looking for an enthusiastic and patient-focused Clinical Pharmacist to join Mansfield North Primary Care Network, supporting approximately 60,000 patients across five GP practices. This is an exciting opportunity to work as part of a multidisciplinary team delivering high-quality medicines optimisation, long-term condition management and clinical medication reviews across a variety of primary care settings, including GP practices, care homes and patients' homes. You will work closely with GPs, Pharmacy Technicians, Nurses and other health and social care professionals, while receiving support and mentorship from experienced Senior Clinical Pharmacists and Neighbourhood Lead Pharmacists. The role also offers the opportunity to further develop your clinical expertise and progress towards becoming an Independent Prescriber if not already qualified. Main duties of the job KEY RESPONSIBILITIES Deliver patient-facing clinical medication reviews and long-term condition clinics, supporting patients with complex polypharmacy, improving medicines optimisation and helping to achieve better health outcomes. Provide expert medicines advice to patients and healthcare professionals, supporting prescribing processes, medicines reconciliation, repeat prescribing, discharge management and safer prescribing initiatives. Lead and contribute to quality improvement projects, audits and PCN workstreams while supporting Pharmacy Technicians, driving service development and promoting best practice across the Primary Care Network. KEY REQUIREMENTS Master's Degree in Pharmacy (MPharm) and current registration with the General Pharmaceutical Council (GPhC). Minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio Strong knowledge of medicines optimisation, prescribing principles and long-term condition management, with the ability to make evidence-based clinical recommendations. Face to face support for the technician within the neighbourhood and close working with other PCN pharmacists within the neighbourhood and wider PCN. Patient facing long-term condition clinics See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g., COPD, asthma). Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e., medicines optimisation). Make appropriate recommendations to Senior Pharmacists or GPs for medicine improvement. Patient facing clinical medication review Undertake clinical medication reviews with patients and produce recommendations for Senior Clinical Pharmacist, Nurses and/or GP on prescribing and monitoring. Patient facing care home medication review Undertake clinical medication reviews with patients and produce recommendations for the Senior Clinical Pharmacist, nurses or GPs on prescribing and monitoring. Work with care home staff to improve safety of medicines ordering and administration. Patient facing domiciliary clinical medication review Undertake clinical medication reviews with patients and produce recommendation
