The Vocational Nurse provides nursing care to patients in an assigned clinical area under the supervision of a Nurse Supervisor. The LVN applies clinical nursing knowledge and demonstrated skills to ensure the safety and comfort of patients and families according to legal, organizational, and professional standards. The Hospitalist LVN works closely with hospitalist physicians, Registered Nurses, Case Management, and other members of the interdisciplinary team to support the coordination of care for hospitalized patients and promote continuity of care throughout the hospitalization. Primary Responsibilities: Patient Care Coordination: Work closely with hospitalist physicians; facilitate communication; coordinate orders, diagnostics, consultations, admissions, and follow-up needs; support patient flow Clinical Nursing Responsibilities: Obtain and document patient information; communicate patient changes; administer medications within scope; assist during emergencies Admissions, Discharges and Transitions of Care: Assist with direct admissions, transfusion admissions, discharge coordination and continuity of care Patient Education: Reinforce patient and family education; identify learning needs; reconcile medications and update pharmacy information Documentation and Compliance: Maintain Epic documentation; comply with HIPAA, OSHA, PHS, EHS, infection prevention and safety standards Customer Service and Teamwork: Deliver compassionate service; collaborate with interdisciplinary teams; maintain professional telephone etiquette Quality Improvement and Department Support: Attend meetings and education; participate in quality initiatives; monitor supplies and equipment; report unsafe practices You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Graduate from an approved vocational nursing program Current and valid Texas LVN license; BLS through the American Heart Association 5+ years in ambulatory/primary care setting Proven ability to use equipment and related supplies for the assigned patient population and clinical area Proven excellent communication and coordination skills Preferred Qualifications: 2+ years inpatient hospital experience Experienced in EMR documentation practices and Epic use Proven ability to work collaboratively with interdisciplinary healthcare teams Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $24 - $43 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
