Job description

Job summary We have an exciting new opportunity to join our team as a Practice Pharmacist. The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription requests, while addressing both the public health and social care needs of patients in the GP practices that make up the PCN. The post holder will act within their professional boundaries, supporting and working alongside our medicines management team. In this role, you will be supported by our Practice Pharmacist who will manage and mentor them. As part of a multi-disciplinary team, the post holder will take responsibility for areas of chronic disease management and long term conditions management within the practice, undertaking clinical medicines reviews and address public health and social needs of our patients. Main duties of the job Provide expertise in clinical medicines review & address public health & social needs of patients in GP practices Reduce inappropriate polypharmacy & wasteful prescribing Reconcile medicines following hospital discharge & work with patients & community pharmacists to ensure patients receive the medicines they need post discharge Contribute to reductions in medicine related hospital admissions & readmissions by supporting patients to get the best outcomes from their medicines, identifying & addressing medicines related issues Interface with community & hospital pharmacy colleagues & develop referral processes between primary care professionals including the promotion of the repeat dispensing service Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions & medicines reaching review dates Manage patients & health care professionals medicine queries Increase quality & safety of prescribing through mechanisms such as audit & PDSA cycles Manage practice formularies to improve the quality, safety, & cost effectiveness of prescribing Implement drug withdrawals & alerts eg, MHRA aimed at improving medicines safety Work with primary care professional & patients to implement NICE & other evidence-based guidelines Contribute to multimorbidity reviews & MDT reviews Provide medicines information & training to Practice healthcare professionals & admin staff. Review Pathology results for patients on known medicines.
