Job description
Job summary We are seeking a highly motivated and experienced Pharmacy Technician to join our friendly and forward-thinking GP practice team. This is an exciting opportunity to play a key role in supporting the safe, effective and efficient use of medicines, working collaboratively with GPs, clinical pharmacists, nurses and the wider multidisciplinary team to deliver high-quality patient care. The successful candidate will be a registered Pharmacy Technician with relevant primary care or medicines management experience. You will be responsible for supporting medicines optimisation, medication reviews, prescription management processes, medicines reconciliation, clinical system audits, and responding to medication-related queries from patients and healthcare professionals. Main duties of the job You will support the delivery of high-quality, safe and effective medicines management services within the practice. Working closely with GPs, Clinical Pharmacists and the wider multidisciplinary team, you will help optimise prescribing, improve patient outcomes and ensure medicines are used safely and efficiently. Key responsibilities include medicines reconciliation following hospital discharges and outpatient appointments, supporting medication reviews, managing prescription and medication queries, and processing repeat prescriptions in line with practice protocols. You will undertake clinical audits and system searches to support prescribing safety, QOF and quality improvement activities, monitor and action medication safety alerts and MHRA guidance, and support monitoring and recall systems for patients receiving long-term medications. The role also involves liaising with secondary care providers, community pharmacies and other healthcare professionals to resolve medication-related issues, maintaining accurate patient records, ensuring compliance with information governance and medicines management policies, and contributing to service development initiatives that enhance patient care and operational efficiency. The successful candidate will be GPhC registered, demonstrate excellent communication and organisational skills, possess strong attention to detail, and be committed to delivering safe, effective and patient-centred care within a busy primary care environment.
