Paramedic for Coco PCN

Job summary The Coco PCN Home Visiting Paramedic is an exciting opportunity for an experienced and motivated Paramedic to make a meaningful difference to patients across the local community. Working across patients homes and GP practices, you will provide high-quality, compassionate and patient-centred care, using your clinical expertise to assess, diagnose, treat and support patients with a wide range of health needs. You will work closely with GPs and the wider multidisciplinary team to ensure patients receive the right care, in the right place and at the right time. This role would suit someone who thrives on making a difference, is passionate about improving patient outcomes and enjoys working collaboratively with colleagues across primary, community and wider health and care services. Main duties of the job Key Responsibilities Undertake comprehensive clinical assessments, examinations, diagnosis and treatment. Provide high-quality home visits to patients unable to attend the practice. Develop and review individualised care and treatment plans, including appropriate investigations and follow-up. Recognise and appropriately manage acutely unwell or deteriorating patients, escalating care when required. Work collaboratively with GPs and the wider multidisciplinary team to provide coordinated care. Support patients and families through clear communication, education and advice. Make appropriate referrals and onward care arrangements. Maintain accurate and timely clinical records. Participate in professional development, training and quality improvement. Work within all relevant clinical, safeguarding, confidentiality, health and safety and professional standards. The role combines home visits and practice-based clinics, assessing, diagnosing, treating and appropriately referring patients to provide safe, effective and timely care. Working alongside GPs and the wider multidisciplinary team, you will support urgent and routine care, improve access to primary care, reduce avoidable hospital admissions and improve patient outcomes.
