Senior Mental Health Practitioner

Job summary We are seeking a motivated and compassionate Registered Mental Health Nurse (RMN) to join our integrated Community Mental Health Team in Portsmouth. In this rewarding role, you will manage a complex caseload of adults with severe and enduring mental health conditions, acting as Lead Professional to coordinate care, deliver recovery-focused interventions, undertake comprehensive assessments, and support service users to achieve their goals. Working within a multidisciplinary team, you will play a key role in risk assessment and management, care planning, safeguarding, service improvement, and partnership working with service users, carers, and external agencies. You will also contribute to the development of colleagues through mentorship, supervision, and collaborative working. The ideal candidate will be an NMC-registered RMN with experience supporting people with acute or severe mental illness, excellent communication skills, and a passion for delivering high-quality, person-centred care that promotes recovery, choice, and independence. Main duties of the job Manage and coordinate a caseload of adults with severe and enduring mental health conditions. Act as Lead Professional, developing and reviewing person-centred care plans and risk management plans. Undertake comprehensive mental health, risk and needs assessments. Deliver recovery-focused therapeutic interventions that promote independence, choice and wellbeing. Work collaboratively with service users, carers, community services and multidisciplinary teams to ensure effective care coordination. Maintain accurate clinical records and contribute to service performance and outcome monitoring. Prepare professional reports and attend multidisciplinary meetings, reviews and case conferences. Provide mentorship, support and supervision to students, trainees and junior staff. Participate in safeguarding, clinical audit, quality improvement and service development activities. Ensure care is delivered in line with relevant legislation, professional standards, Trust policies and best practice.
